二、申请人的资格要求：

3.本项目的特定资格要求： 1)投标人在中华人民共和国境内外注册且具有独立的法人资格，能提供上述产品及相应服务的代理商或生产厂家；1) the bidder registered within the territory of the People's Republic of China and has independent legal person qualification, can provide the products and corresponding service agents or manufacturers;
2)必须提供所投产品的生产商针对本次招标项目出具的独家授权书，投标设备应具备有效期内的有关行政管理部门颁发的医疗器械注册证（备案证）；2) must provide the cast product exclusive authorization letter issued by the producer for the project subject to tender,the equipment shall have the medical device registration certificate (record certificate) issued by the relevant administrative department within the validity period.;
3)具有招标文件中所需设备的供货和售后服务的能力；
3) with the supply of equipment required in the tender documents and the ability to after-sales service;
4）参加本次招标活动前3年内，投标人在经营活动中没有违法记录，无利用不正当竞争手段骗取中标，无行贿犯罪记录；

5) 投标人开户银行在开标日前三个月内开具的资信证明原件或复印件。
